Late Registration for the 2011 Fall Season. Deadline

All forms must be received no later then May 31st,
2011.
Registration was held Friday May 7th, Saturday May
8th and late registration was Wednesday May 11th at
the Blue Valley Farm Show.
A late fee 0f $10 will be charged per player.
PLAYERS: Must be between the ages of 5 and 16 as
of August 1, 2011.
FEE: The registration fee is $60 plus $10 late fee for
each Travel Player and $45 plus $10 late fee for 1st
In-house Player; $40 plus $10 late fee each additional
child playing In-house. Each parent is required to
work 1 hour in the concession stand per child. Must
have a copy of child's Birth Certificate (new players
only).
SIGNS-UPS ARE FOR:
Intramural Co-ed
Travel teams: U-10, 12, 14, 16 for girls and U-10, 12,
14, 16 boys (subject to change)
Coaches clinics
Referees
Sponsors
Soccer Camp (July 18-22)
Coaches
VOLUNTEERS ARE NEEDED FOR: Coaching, Referees,
Field Maintenance and concession stand committee.
Training, equipment, mentors and assistance will be
available to all who volunteer. Come "Have a Blast"
with the kids!
